Abstract

Carotid-cavernous sinus fistula is a rare situation resulting from severe craneofacial trauma. The clinical presentation is a consequence of retrograde arterialized flow of the venous system: pulsating exophtalmos, proptosis, orbital bruit, diplopia as result of cranial nerve compression; and occasionally monolateral midriasis caused by the disruption of parasympathetic innervation. Differential diagnosis is made with orbital apex syndrome, superior orbital fissure syndrome, cavernous sinus thrombosis, orbital haematoma or tumor, and others. Computerized tomography witb contrast enhancement is orientative, but only the arteriography is patognomonic. We analize a clinical case associated with mandibular fracture, satisfactorily resolved with wire coils embolizations.
