Abstract

The Home Gerontologic Integral Support Program (AGAD) is addressed to people with disability and their caregivers. The main objective of this project is to offer a different and innovative approach in the social attention of elderly people with dependence at their own homes, promoting support to caregivers and avoiding institutionalization of the patient. The Home Gerontologic Integral Support Program (AGAD) offers different types of care at home, far from the welfare model of Home Help, and is based on the integral caring principle provided from different professional perspectives. Thus, to offer a multidisciplinary support team in geriatric home support is a key and innovative point in the development of this project.
