Abstract

The clinical history is the essential document of medical learning and medical practice. The first complete histories can be found in the book The Epidemies I and III of the Corpus Hipocraticum. It was continued in The Middle Age with the Consilea and during the Renaissance, it received the name of Observatio. It was perfectionated by Sydenham and it was completed through of XVIII century with the anatomo-clinic ethodology and in the XIX with the development of phisiopathological technical. The complexity of the clinical history suffered a fast increase though the XX century when it turns into multidisciplinary and of forced fulfillment. The computerization of the clinical history can suppose radical changes in the XXI century.
