Sífilis secundaria que debuta con hipertrofia del anillo de Waldeyer y otitis serosa

Abstract

Syphilis is an infectious disease caused by the Treponema pallidum with three clinical stages and may present various ENT manifestations, mainly at the secondary stage. Despite the advancements in medicine toward prevention, diagnosis and treatment, syphilis remains a public health problem worldwide, specially due to an increase in HIV transmission rate, specially among homosexual population. We report a case of secondary syphilis revealed by skin rash, otitis media with effusion and a Waldeyer´s ring hypertrophy that at first, was suspected of malignant proliferation. Due to an unfavorable evolution despite of the medical treatment, we decided to undergo surgical biopsy of the adenoid pad. The sample was reported as “inflammatory infiltrate with great amount of plasmatic cells”, and the Pathologist suggested us to search for TPHA-VDRL titre, that finally resulted positive. The patient received a penicilin G intramuscular doses and the clinical case resolved over a period of few weeks.
